For two years, I practiced across several skilled nursing facilities, including Fairview Nursing Care Center, South Shore Rehabilitation & Nursing Center, and Park Avenue Extended Care, all under a structured supervision model with collaborating doctors’ oversight. I carried real clinical responsibility for medically complex subacute and long-term care patients: managing multimorbidity, post-acute rehabilitation, goals-of-care conversations with families in their hardest moments, and the documentation and regulatory standards unique to the SNF setting.
